Consultation Requests Through Meta Instant Forms

About this policy
Mi Piel Clinic respects your privacy. This policy explains how we handle personal information submitted through our consultation-request forms on Facebook and Instagram, known as Meta Instant Forms, and related administrative follow-up.
Information collected during a later assessment or treatment may require additional privacy information and consent.
Information we collect
We only collect information reasonably necessary to review and respond to your consultation request.
When you submit a consultation request, we collect:
• Your name, email address and phone number.
• Your preferred contact method and time.
• Your answers to commercial, non-medical questions, including availability, preferred consultation time, interest in discussing a full protocol and willingness to learn about home care.
We do not request medical information through this form. Please do not submit medical history, diagnoses, medications, allergies, medical photographs or other sensitive health information. Any treatment suitability questions will be reviewed privately with Richard outside the Meta form.
How we use your information
We use your contact details and responses to review your consultation request and contact you by your selected method to discuss availability and next steps.
Submitting the form does not confirm an appointment, establish treatment suitability or provide consent to treatment. Your preferred time remains a request until our team confirms it.
We do not treat your consultation request as permission to add you to unrelated promotional lists. Any additional use requiring consent will be explained separately.
Your choices and withdrawing contact consent
Submitting the form is voluntary. By submitting a request, you ask Mi Piel Clinic to contact you about that request using your selected method.
You may withdraw your permission for further contact by calling or emailing us at the details below. Please identify the contact details used in your request so we can locate it. You do not need to provide medical information.
Withdrawing permission may prevent us from completing your inquiry. It applies to future contact and does not reverse uses already made with your consent or require deletion of records we must retain for a lawful reason.
Who can access your information
Access is limited to authorized Mi Piel Clinic team members and necessary service providers supporting the form, scheduling, data storage and communications. We limit access and sharing to what is needed for those purposes.

We do not sell personal information. We do not upload patient lists or clinical information to Meta to create or target advertising audiences.
We may disclose information when required by law. Other disclosures requiring your consent will be explained before we ask for that consent.
Security, storage and retention
We use reasonable safeguards appropriate to the information to reduce the risk of loss, misuse and unauthorized access or disclosure. No online form, email or storage system can be guaranteed completely secure.
Meta and other service providers may process or store information outside Canada, where it may be subject to local laws and lawful access by authorities.
We retain inquiry information only as long as reasonably necessary to handle your request and related follow-up, meet applicable legal obligations or address a complaint or dispute where retention is justified. When information is no longer needed, it is securely deleted or anonymized, subject to applicable requirements.
If your inquiry becomes an appointment or treatment record, additional clinical documentation and retention practices may apply.
Access, correction and deletion
You may contact us to request access to the personal information we hold about your inquiry, correct inaccurate information or request deletion.
We may request limited information to verify your identity and locate your records. We will respond within the time required by applicable law and explain any permitted limitation or refusal.
Deletion may not be possible immediately where information must be retained for legal obligations or an unresolved complaint. We will explain any applicable reason. For information held independently by Meta, its own privacy-request process may also apply.

Please do not include medical information in your initial privacy inquiry. If further details are needed, we will explain an appropriate way to provide them.
You may also raise concerns with the applicable privacy regulator, including the Office of the Privacy Commissioner of Canada or, for matters within its jurisdiction, the Information and Privacy Commissioner of Ontario.
Changes to this policy
The published policy will show its effective date. If our practices change in a way that requires new consent, we will obtain it before using your information for that new purpose.
